U.S Army Medical Research Unit: Improving malaria diagnosis in Africa, one lab at a time

OYUGIS, Kenya – Inside Rachuonyo district hospital, Simba Mobagi peers through his laboratory’s only microscope at a sick woman’s blood sample.

The 33-year-old laboratory technologist’s goal – rapidly identifying malaria parasites.
Dozens more samples await his eyes. Each represents a patient suffering outside on wooden benches.

Mogabi takes little time to ponder his workload. He quickly finds malaria parasites, marks his findings on a pink patient record and moves to the next slide. Much to his surprise, a U.S. Army officer arrives, removes his black beret and sets down a large box.

Inside Maj. Eric Wagar’s box is a new microscope – a small gesture within U.S. Army Medical Research Unit-Kenya’s larger efforts to improve malaria diagnostics in Africa.

For more than 40 years, USAMRU-K – known locally as the Walter Reed Project – has studied diseases in East Africa through a partnership with the Kenya Medical Research Institute.

Wagar heads USAMRU-K’s Malaria Diagnostics and Control Center of Excellence in Kisumu, a unique establishment begun in 2004 that’s since trained more than 650 laboratory specialist to better their malaria microscopy skills.

“Working with the Walter Reed Project is so good for the community, as it benefits the patient,” Mobagi said, who is looking forward to attending the center’s malaria diagnostics course. “Plus, having a new microscope improves our work environment. Work will be easier and we will have better outcomes.”

Back in Kisumu, wall maps mark the center’s success, with hundreds of trained lab technicians from more than a dozen countries across the African continent. International students have come from Ireland, the U.S. and Thailand.
Many students are sponsored through U.S. government aid programs aimed at reducing disease in Africa or by nongovernmental organizations. Most of the center’s 0,000 annual budget comes from the U.S. President’s Malaria Initiative. Other funding is from the U.S. Defense Department, NGOs and pharmaceutical companies.

For students to practice malaria identification, five Kenyan lab technicians work tirelessly to create a variety of blood specimens. Slides may show one or more of malaria’s several species – others are free of parasites. The majority of malaria cases are the falciparum species, but many people are co-infected with other species and it’s important for students to recognize that, Wagar said.

“At our course, lab students learn skills and habits that increase their ability to accurately detect malaria on blood slides. Yet, when they return to their local laboratories, they face the challenge of changing habits and procedures,” Wagar said. “Changing behavior is hard to do.”

In late-April, Wagar accompanied Jew Ochola, 28, the center’s daily operations manager to Oyugis, the district center of Rachuonyo that lies roughly 30 miles south of Kisumu in Kenya’s Nyanza province.

“First I do an assessment of the hospital’s lab, what procedures they have, the number of people on staff and the equipment they use,” Ochola said. “By partnering with laboratory managers, we hope to increase standards and improve efficient and effective diagnosis.

The goal is to lessen the burden of malaria on the local people.”

To mark progress, lab staffs must collect 20 slides each month that show properly handled blood samples. Monthly visits will mark performance improvement.

Through quality malaria diagnosis, USAMRU-K is part of a larger public health effort to reduce malaria’s impacts on Kenyan’s lives. Illness means paying for treatment and less wages earned, creating an impact on the economy.

“By mitigating a public health burden, people should have more time to grow food and have money for things other than medical care,” Wagar said. “We can’t expect to see change right away, but hopefully things will be a little bit better every month.”

Working with the Djibouti-based Combined Joint Task Force – Horn of Africa and other DoD agencies, the center recently offered microscopy courses through U.S. military partnership events in Ghana, Nigeria and Tanzania. The effort supports U.S. Army Africa’s strategic engagement goal of increasing capabilities and strengthening capacity with the militaries of African nations, Wagar said.

“To date, that includes eight Kenyan military lab techs, 17 from the Tanzania People’s Defense Force and 30 Nigerians,” Wagar said.

Accurate diagnosis is also a key factor for military readiness, Wagar said. For example, a Kenyan soldier stationed in Nairobi – where malaria is less prevalent – is susceptible to the disease if posted elsewhere in the country.

“Improving malaria diagnosis within African military laboratories sets conditions for healthier troops,” Wagar said. “When forces are healthy, they are more capable to support their government and regional security.”

HE’S BUSY ERASING DEMOCRATIC PROCESS AND WITH IT DEMOCRACY IN THE UNITED STATES

The Entitled Party

President Obama and the left wing of the Democratic Party think they are entitled to win. From our narcissistic President to screaming union organizers, they are puffed up with self-righteous zeal. They must have health care to save the sick, they must shut down Louisiana oil rigs to save the planet, they must defend government unions to save the middle class.

Of course, each side thinks they are right. Being right is no excuse. You have to abide by the law, you have to abide by elections, you have to respect the courts and constitutional separation of power, or else we no longer live in a democratic country. In our democracy, no one is entitled to win. If you won’t lose, you cannot have democracy.

What you have are the Wisconsin Democrat senators who are unwilling to abide by the election results that put them in a minority. What you have is Reid and Pelosi, ramming Obamacare through by breaking rules of procedure, in order to flout the 2010 election results. What you have is the Obama White House, blocking Congress’s right to confirm appointees, and openly ignoring federal courts. What you have is the Justice Department announcing it will no longer defend the 1996 Defense of Marriage Act in court, as if Obama gets to decide which laws are constitutional. What you have is a Democratic Party run amok, undercutting our democracy in the service of their own power.

The complacency, nay, the vociferous support, from Democrat leaders and the legacy media for this disregard for the rule of law reminds me of the old joke about the psychiatrist. A man is sent by his family to see a shrink because he thinks he’s a chicken. After months of treatment, he is still clucking. The family asks the psychiatrist if he’s told his patient he is not a chicken. "No," the psychiatrist admits. "Why not!" "Because I like the eggs."

The Democrats like the eggs. They like imposing their will, whether it be ObamaCare, or the off-shore drilling moratorium, or the blockage of Wisconsin’s elected government. Are they really this short-sighted? Don’t they understand the damage to our democratic system by these anti-democratic precedents? Do they really want to change congressional rules so that the House and the Senate version of bills no longer have to be reconciled, as they did to jam ObamaCare through by the fiction it was a finance bill? Do they really want the Interior Department ignoring federal court orders? Do they really want state senators refusing to accept that when you lose an election, the other side gets to pass their agenda?

Obama appointed extremists for important administrative positions, controversial and even creepy people, like Van Jones, whom he knew would not get past Congressional confirmation. The checks and balances between executive and legislative branch were instituted by our founders for this exact purpose. The executive nominates but Congress must confirm — bedrock principles of American democracy. Obama’s answer: flout the law. Call his appointees ‘czars’ and bypass confirmation. This is not legal and it is not democracy. Do the liberal legacy media and Obama’s fellow Democrats want presidents to have this unlimited power? Do they really want to give up the safeguards of congressional confirmation by calling appointees czars?

Czars indeed.

The White House is not only ignoring elections and subverting the power of Congress, it is also willing to disobey federal courts. When the health care bill was challenged in court and the administration lost, Obama ignored the ruling of Justice Roger Vinson of the U.S. District Court in Florida. Judge Vinson declared the entire ObamaCare bill unconstitutional in a ruling that the judge stated was the equivalent of an injunction. The White House has not halted implementation. The White house has not followed normal rules to fast-track the appeal process so the Supreme Court can decide. Our White House seems entirely comfortable to show contempt of court.

In Louisiana, the administration didn’t like a court ruling lifting the moratorium on off-shore drilling, so what did the Obama administration do? It ignored the court. In response, on February 2, the U.S. District Court Judge Martin Feldman held the Department of Interior in contempt. The Administration then adopted a go slow policy and did not issue a single permit. So on February 21, Judge Feldman ordered the Obama administration to act on five deep water drilling permits in the Gulf of Mexico within 30 days, calling the delays in issuing new decisions "unreasonable, unacceptable, and unjustified." We have a White House that places its anti-energy policy above the rule of law. This is unacceptable in a democracy.

Democracy is a complex system based on cultural norms and principles as much as institutions. As we see governments topple in the context of resurgent jihadi movements in the Arab world, we are keenly aware that elections alone rarely lead to democracy. George Washington was an almost unique figure in the history of the world, in that he relinquished power. Our founding fathers were political geniuses who gave us a system of checks and balances to curb misuse of power by those who govern. As Americans, we are privileged to witness the recurring, orderly transfer of power from one administration to the next, through which voters get to determine the direction of their government and correct mistakes and imbalances.

We are seeing in both the Obama White House and the Wisconsin Senate that the Democratic Party is unwilling to lose. Over and over in the past two years, we have seen a Democrat administration willing to flout the courts, flout rules and regulations, and flout the voice of the people as expressed in elections.

Disregard for the democratic limits on power is as important as the administration’s fiscal irresponsibility that threatens our prosperity, as important as the explosive growth of bureaucracy that threatens our liberties.

Our democracy cannot survive if only the Republican Party cares about it. It is time for centrist Democrats to throw off the power grab by the radical wing of their party and start defending the Constitution, as they have sworn to do.

What is a Plant Based Diet?
This question gets asked a lot ,and many people have different ideas of what a plant based diet is and so I intend to find out for myself what its really all about.
A good place to look for answers is wikipedia , so I went there to see what this online dictionary had to say , and the answers may surprise you?

According to Wikipedia a Plant Based Diet can mean different things to different people.
Here are the examples of a plant based diet.

Seven variations are mentioned ,but if you combine and mix them with your cultural cuisine then you could potentially have dozens of variables.

These are the seven listed:

HERBIVORE: can be any mammal or human that adapts to eating plants and not meat of any kind.

VEGANISM: Is the ultimate 100 percent plant based diet ,with absolutely no food from animal sources including the bee product honey. How ever probiotic bacterial cultures are generally accepted.

FRUITARIANISM: Is very similar to Veganism , where the meals consist of primarily fruit and their seeds.

RAW VEGANISM: Is a type of Veganism, but with food uncooked in their RAW state with life giving Enzymes , and water soluble vitamins like vitamin C in-tacked and ready for available digestion of the nutrients. Some of their raw food preparations may be dehydrated at very low temperatures that may take hours to prepare to preserve the enzymes of the food. Raw fermented foods are also used.

VEGETARIANISM: Is a plant based diet that may include eggs,milk and cheese.
One important note on Vegetarianism: Many commercially processed foods are labeled as vegetarian as the Processed Food Giants know that this is a huge and growing niche consumer market place for them.

MACROBIOTIC DIET: Macrobiotic cuisine originated in japan ,then expanded world wide .
This diet is plant based and uses all whole foods from nature including vegetables, fruits, legumes,seeds, fermented soy products,sea vegetables, mushrooms, bancha green tea, and other herbs with occasional sea food mostly fish.

SEMI-VEGETARIANISM: Is a plant based diet with occasional meat products from land and the sea.

If you could label your self what would you be?

For myself I would say that I am a combination of some of the above and I practice the following from the most practiced to the least practiced in this order.
Raw Veganism ,Veganism,Fruitarianism ,(Often I eat only fruit at a sitting)
Macrobiotic diet,( I live in South East Asia and use local organically grown and wild crafted produce in season as much as possible with a Thai flavor) I am also a Sproutarian. A process of adding air oxygen and pure water in darkness to germinate seeds , nuts, and grains. This process will Alkalize , and increase the bio available nutrients for my body, and gives me the opportunity to save the seeds for my garden.

I practice Vegetarianism the least . Wild same day seafood from my local Sea Gypsy village. I include on occasion , a free range organic duck or hen egg. Some times on occasion a free range duck or hen that is found in my village. Maybe on occasion some raw organic goat milk that I make cultured Kefir from.
Note that I boycott most all man made processed foods , I look for real food that only nature can provide.
I do not smoke , and I do not take any Pharmaceutical Drugs , including over the counter prescriptions.
I do on occasion drink Organic Red Wine with a meal that includes a small portion of cooked meat with lots of raw vegetables with steamed and cultured foods.
When you prepare your meals at home you are in control of your diet . When you eat out you are out of control of your diet.

For Bill Clinton what kind of Plant Based Diet diet would he be on? The answer will surprise you.

The best choice to have , is to eat foods loaded with life sustaining enzymes like RAW LEAFY GREENS, FRUITS,VEGETABLES,SPROUTED GRAINS,NUTS,SEEDS,HERBS,and Sea vegetables, with the following foods for your Vegetarian Plant Based Diet Plan.

Steamed , baked, or pan fried small cold water fish, such as Wild Seafoods of ,Salmon,Tuna,Mackerel, Sardines,and smaller Tuna directly from the wild , and never from the farm.
Farm raised fish , are penned in and their movement is controlled in close proximity to each other causing stress and disease. Routinely the Fish farms will dose the water with a chemical bath ,including antibiotic treatments.This high protein and essential vitamin B-12 source will also provide essential bioavailable, Omega 3 fatty Acids that the body needs.But many people will think about all the Mercury ,that we hear about all the time from the media.The fact is a person will have much more Mercury in their mouth than a fish would have in their bodies ,if the persons mouth is full of dental mercury amalgam fillings that is getting a lot of press lately.
I recommend to find a good Environmental Dentist and have the toxic fillings removed.
To reduce or even eliminate your exposure to Mercury stay away from large predatory fish like sharks, swordfish ,Large Tuna, and other large predatory seafood species as the mercury load will be much greater.
A good tip to reduce or eliminate Mercury or any heavy metal from consuming fish is to drink Green Tea with the meal. Yes its true .Studies have proven that drinking Green tea with fish creates a chelation effect that only green tea can do, as black tea does not work as well according to the studies. The reduction of mercury can be as high as 95 percent in some of the results. Eat sparingly and in small portions.
If the ducks, chickens, and their eggs are grass and grained fed organically and open range then this food may be considered in your diet. Eat sparingly and in small portions.
Dairy that is grass feed ,organic,and cultured naturally, is good in moderation and sparingly.
Many people may think ,how can I eat this way it looks bland,boring,not tasty,and to difficult to make? This kind of thinking is far from the truth , as many large groups of people today living on planet Earth are eating exactly this way every day of their lives and living happy and healthy to a ripe mature age.

These sociable ,happy people lived close to Nature and Spirit, with a strong sense of purpose in life. Many of them lived on into their Hundredth Birthday and beyond. I live a Plant Based mostly raw diet every day for years now with no problem what so ever , and you can do it to, if you want to ?

To go from the SAD diet, (Standard American Diet )like I did, you would have to slowly transition over into it and I will explain why and in much more detail in another article because,for most people living in the modern world it will not be so easy in the beginning . Some people may fail their first time around especially if they go into it fast and abruptly. They may only succeed the second or third time around . This can happen for a host of different reasons. But for those who have succeeded in the transition over to a plant based diet with lots of living foods will know how they felt then and how they feel today over time.

Their taste buds have changed ,among many other things over time and when they do cheat and go back to their old ways of dieting its not the same as before. Trust me on this one. After eating for years Natures Plant based foods of Natures natural sugars and continuing to do so to this day,I can only give you one incident that under unusual circumstances of myself eating into a sugary processed cookie . It only confirmed the fact in my mind and to my taste buds as well, that there is no longer any appeal anymore to my appetite or even to my palate and taste buds to even want to entertain the idea of going back to consuming processed commercial foods again that is lacking in fiber and real taste that Nature intended.

Sarah Palin was right.

John Boehner — make that Speaker-elect of the House John Boehner — was right.

While Americans were busy celebrating with family and friends and presumably not paying attention to the news, the New York Times, in a story ironically dated Christmas Day — a holiday celebrating the birth of the Prince of Peace — reported the following:

Obama Returns to End-of-Life Plan That Caused Stir

WASHINGTON — When a proposal to encourage end-of-life planning touched off a political storm over "death panels," Democrats dropped it from legislation to overhaul the health care system. But the Obama administration will achieve the same goal by regulation, starting Jan. 1.

In other words, the 2009 charge leveled by former Alaska Governor Sarah Palin and the then-House Minority Leader Boehner that Obama fully intended to set up what Palin termed government "death panels" — panels that Boehner said would set the government on the road to euthanasia — is no longer a charge.

It’s reality. By executive fiat — in this case a new Medicare rule issued by Obama Medicare chief Dr. Donald Berwick.

Palin, who made the charge on her Facebook page on August 7, 2009 during the health care debates, came under a fusillade of scornful and demeaning political attacks from political opponents after pointedly saying this about the prospect of death panels:

And who will suffer the most when they ration care? The sick, the elderly, and the disabled, of course. The America I know and love is not one in which my parents or my baby with Down Syndrome will have to stand in front of Obama’s "death panel" so his bureaucrats can decide, based on a subjective judgment of their "level of productivity in society," whether they are worthy of health care. Such a system is downright evil.

Her famous sharp criticism was enough for the plan to be quickly dropped by Congress.

Now, with Americans absorbed in a festive holiday and ignoring Washington momentarily, the Obama administration has found a way to achieve its death panel goal anyway, as the Times now admits. Says the paper of the new Christmas death panel regulation that replaces medical science and voluntary private judgment with the inevitable pressure of politicized health care :

Congressional supporters of the new policy, though pleased, have kept quiet. They fear provoking another furor like the one in 2009 when Republicans seized on the idea of end-of-life counseling to argue that the Democrats’ bill would allow the government to cut off care for the critically ill.

Which is another way of saying something else:

Governor Palin has been vindicated. Speaker Boehner has been vindicated.
And Palin’s critics in particular now have more than holiday eggnog all over their faces. Obama’s Dr. Berwick has re-ignited one of the most hotly controversial issues of the entire health care debate just as a conservative ascendancy prepares to take power in the next Congress. With no less than Boehner himself taking the gavel from Nancy Pelosi as the new Speaker of the House.

What does this new rule say and do, exactly?

It inserts the federal government in end-of-life planning, precisely as Palin said was Obama’s intention. Not, as was true of its original legislative formulation, every five years. But annually. No one of any sense objects to an individual and doctor having end-of-life discussions about living wills and such whenever they wish. Only the Obama administration and its obsession for control wants the government to incentivize the issue so that doctors must raise it annually, a system that on its face pressures the most deeply vulnerable of Americans in the most Orwellian of terms to end their lives.

Control and pressure. Pressure and control. This is the only two-step philosophical/political dance liberals know. It is, as it were, primal. And the Berwick Medicare rule, constructed in secret and released on Christmas Day when it no one is looking, is a perfect example — if hardly the only example — of how the Obama Administration views its role. Control and pressure. Pressure… and control.

Versus the conservative concept (shorthand version) of liberty and freedom.

Says the Times of the Obama Administration’s justification for its secretive move to mandate death panels by regulatory fiat:

In this case, the administration said research had shown the value of end-of-life planning.

Research? What research could possibly justify a government-sponsored annual attempt to pressure a poor, disabled, or elderly American into believing that they would be better off dead because they’re costing society too much money?

British research. Says the Times:

"Advance care planning improves end-of-life care and patient and family satisfaction and reduces stress, anxiety and depression in surviving relatives," the administration said in the preamble to the Medicare regulation, quoting research published this year in the British Medical Journal."

You read that right.

British research is being cited in the preamble of this Medicare death panel rule as a justification for the new rule — a stunning turn of events that will surely launch a firestorm over trying to remodel the American health care system after the hotly criticized British health care system. A system that makes no pretense of politically rationed health care.

Part of the furor launched over Palin’s remarks was the discovery by millions of frightened Americans that Obama health care bureaucrats admired the British health care system — where the government in fact rations health care on a political basis and decides who should live or die based on what is called the "QALY" — Quality-Adjusted Life Year. This has been discussed previously in this space — in fact just over a week before Governor Palin wrote her Facebook statement. It has also been discussed by health care consultant David Catron here where he explained how the QALY system worked.

In Catron’s words: "A year of perfect health, for example, is given a value of 1.0 while a year of sub-optimum health is rated between 0 and 1. If you are confined to a wheelchair, a year of your life might be valued at half that of your ambulatory neighbor. If you are blind or deaf, you also score low. All that remains is to assign a specific dollar value to the QALY and, voilà, your life has a price tag."

Princeton’s controversial Dr. Peter Singer, a liberal and big believer in the British health care system, happily related the British politicization of medical decisions in a New York Times Magazine article during all of this, an instance in which "Britain’s National Institute for Health and Clinical Excellence gave a preliminary recommendation that the National Health Service should not offer Sutent for advanced kidney cancer." Why? The government said it was too expensive and therefore simply denied the drug. This in turn led to a furious reaction even from stiff-upper-lip Brits with charges their government was "immoral" and willing to let patients die. Grudgingly, the drug was eventually approved. But not before one angry British woman, whose husband’s life was at stake, angrily asked: "What price is life?"
As this is written the Obama Food and Drug Administration is now taking Americans down this same path, rejecting the breast cancer drug Avastin with what many are citing as unbelievable science — but very believable political concerns that the drug is, in the bureaucrats’ view, too expensive. Thereby inserting the judgment of political bureaucrats for medical science — and the freedom of patients to order the drug. Here’s this from the Heartland Institute:

According to Sally Pipes, president of the Pacific Research Institute, the FDA’s decision is not based on the best outcome for patients but instead on the expense of Avastin, produced by Genentech, which can run as high as ,000 per year for a single patient.

The FDA claims its decision had nothing to do with Avastin’s cost and was based solely on the drug’s medical effectiveness," Pipes said. "This isn’t believable. Every year about 40,000 American women die from breast cancer. Avastin is the last hope for many not to meet that fate. While the drug is costly, it often provides immense benefits to patients.

Somewhere an American woman with breast cancer is surely saying the same thing as her British counterpart: "What price is life?"

Singer had an answer. Really. Said the famous Bioethics professor: "Life as a whole has no meaning. Life began, as the best available theories tell us, in a chance combination of gasses; it then evolved through random mutation and natural selection. All this just happened; it did not happen to any overall purpose."

Thus, since life really has no overall purpose, the government should be in the business of using Medicare to pressure the poor, the disabled and the elderly that — nudge, nudge — isn’t it time to bid the planet hasta la vista?

WHICH BRINGS US TO DR. DONALD BERWICK himself, the Obama administration’s Medicare recess-appointed head of the Center for Medicare and Medicaid Services. Dr. Berwick personally issued the Christmas Death Panel Rule, confirming in spades why he received a recess appointment from Obama. It was clear to Senate Democrats that Berwick’s chances of surviving a Senate confirmation battle were iffy at best. Why? Precisely because Berwick was well on record as expressing his deep admiration — make that lust — for the British government run system, saying: "I am romantic about the National Health Service; I love it. … The NHS is one of the astounding human endeavors of modern times."

So Obama waited until he could skip a Senate debate and vote entirely and just recess-appoint Berwick — who in turn is doing exactly what his record suggested he would do.

Sure enough, the philosophy used by the British is precisely what Berwick used to describe the new Berwick Rule. Reported the Times of Berwick:

"Using unwanted procedures in terminal illness is a form of assault," Dr. Berwick has said. "In economic terms, it is waste. Several techniques, including advance directives and involvement of patients and families in decision-making, have been shown to reduce inappropriate care at the end of life, leading to both lower cost and more humane care."

So.

What do we have here?

• The death panels were written into the original version of ObamaCare.

• Governor Palin, speaking out in her famous Facebook post, pulled back the shroud surrounding this horrifying idea. Less noticed at the time — but undoubtedly a headline grabber now — Minority Leader Boehner agreed, citing alarm over government sponsored euthanasia. Said Boehner:

Section 1233 of the House-drafted legislation encourages health care providers to provide their Medicare patients with counseling on "the use of artificially administered nutrition and hydration" and other end of life treatments, and may place seniors in situations where they feel pressured to sign end of life directives they would not otherwise sign. This provision may start us down a treacherous path toward government-encouraged euthanasia if enacted into law.

Obama protested he had no intention of "pulling the plug on Grandma" — but the idea, embodied in Section 1233 of the House version of the bill, was pulled from the final bill in part because of Palin’s — and Boehner’s — focused attention.

• Obama installs Dr. Donald Berwick to head the Medicare program as a recess appointment because Berwick’s controversial enthusiastic embrace of the British health care system and its death panel procedures would have prevented his confirmation.

• On Christmas day 2010, the Times reports the death panel idea will become a Medicare rule on January 1, 2011 — that would be four days from today. How? By fiat. As a government Medicare "rule" or "regulation" as put forth by the government agency now run by Dr. Berwick. The rule is justified because Berwick believes the government must "reduce inappropriate care at the end of life" A Berwick spokesman says the government should be saying to elderly patients, vulnerable patients, disabled patients — patients like Sarah Palin’s famous Down’s syndrome son: "When the time comes, do you want us to use technology to try and delay your death?" Nudge.

• Elizabeth D. Wickham, executive director of LifeTree, a pro-life educational organization, says of the new rule: "The infamous Section 1233 is still alive and kicking. Patients will lose the ability to control treatments at the end of life."

Oh yes. Did we mention no one was supposed to know about all of this?

Congressman Earl Blumenauer, the Oregon Democrat who wrote the original provision in the House version of ObamaCare that was unmasked by Sarah Palin, has put the word out to his allies. Says the Congressman’s office in an e-mail to his allies:

While we are very happy with the result, we won’t be shouting it from the rooftops because we aren’t out of the woods yet. This regulation could be modified or reversed, especially if Republican leaders try to use this small provision to perpetuate the "death panel" myth.

We would ask that you not broadcast this accomplishment out to any of your lists, even if they are "supporters" — e-mails can too easily be forwarded…Thus far, it seems that no press or blogs have discovered it, but we will be keeping a close watch and may be calling on you if we need a rapid, targeted response. The longer this goes unnoticed, the better our chances of keeping it.

No wonder Blumenauer wants to keep this quiet.

Did you catch that word "us" in the sentence from Dr. Berwick’s spokesperson?

Here’s the sentence again:

When the time comes, do you want us to use technology to try and delay your death?

The word "us" refers not to a doctor and his patient. It refers to the government..

When Obama health care adviser Dr. Ezekiel Emanuel was cited by longtime health care expert Betsy McCaughey as discussing the idea that patients with dementia should be denied treatment, Emanuel’s defenders (he is also the brother of ex-Obama White House chief of staff Rahm Emanuel) floated a Time magazine story saying Emanuel "only mentioned dementia in a discussion of theoretical approaches, not an endorsement of a particular policy." Oblivious to the fact that that no less than the President himself expressed a version of the same sentiment (as has Berwick) when he went on national television to answer a woman’s observation that at over a hundred her mother was very vital with a lot of spirit, and shouldn’t that be taken into account in any government health care decision? Said Obama: "I don’t think that we can make judgments based on peoples’ spirit. That would be a pretty subjective decision to be making. I think we have to have rules…."

Government rules. Like the rule just issued by Dr. Berwick.
A rule that effectively is now going to bully individuals — doubtless many of them poor, disabled or elderly. Ironically creating a system where you will only escape Obama’s government sponsored Big Chill if you are, say, a rich liberal.

In effect the administration is trying to bully Congress by making an end-run with a regulation because Congress said no to Section 1233.

The Heritage Foundation has accurately noted yesterday that, quite aside from the substance here — the new Berwick death panel rule or the FCC’s new net neutrality rules and so on — the real issue is the Obama administration’s clear intent to govern by executive fiat now that it has lost control of the House and, effectively, the Senate as well. Government-by-Obama fiat will be the subject of a furious struggle in the new Congress.

Says Heritage by way of focusing on a return to government by elected officials rather than a central government of rule-making un-elected bureaucrats:

"There is also the Congressional Review Act, which allows Congress to review and overrule regulations issued by government agencies."

Which is to say, the Berwick rule can be undone — if the Congress orders it undone.

This episode is a reminder that Governor Palin took a lot of heat for bringing attention to this issue. No one expects her critics — now proven wrong by Berwick — to give her any credit for being right. Or for that matter to the new Speaker Boehner.

But the fact remains that Palin has shown leadership here — one might call it presidential-style leadership — in persisting with an issue that is now coming back to bite the American people in the form of a new Medicare rule on death panels — reported of all days on Christmas day.

No wonder John Boehner will be Speaker of the House.

U.S Army Medical Research Unit – Kenya: Improving malaria diagnosis, one lab at a time

OYUGIS, Kenya – Inside Rachuonyo district hospital, Simba Mobagi peers through his laboratory’s only microscope at a sick woman’s blood sample.

The 33-year-old laboratory technologist’s goal – rapidly identifying malaria parasites.
Dozens more samples await his eyes. Each represents a patient suffering outside on wooden benches.

Mogabi takes little time to ponder his workload. He quickly finds malaria parasites, marks his finding on a pink patient record and moves to the next slide. Much to his surprise, a U.S. Army officer arrives, removes his black beret and sets down a large box.

Inside Maj. Eric Wagar’s box is a new microscope – a small gesture within U.S. Army Medical Research Unit-Kenya’s larger efforts to improve malaria diagnostics in Africa.

For more than 40 years, USAMRU-K – known locally as the Walter Reed Project – has studied diseases in East Africa through a partnership with the Kenya Medical Research Institute.

Wagar heads USAMRU-K’s Malaria Diagnostics and Control Center of Excellence in Kisumu, a unique establishment begun in 2004 that’s since trained more than 650 laboratory specialist to better their malaria microscopy skills.

“Working with the Walter Reed Project is so good for the community, as it benefits the patient,” Mobagi said, who is looking forward to attending the center’s malaria diagnostics course. “Plus, having a new microscope improves our work environment. Work will be easier and we will have better outcomes.”

Back in Kisumu, wall maps mark the center’s success, with hundreds of trained lab technicians from more than a dozen countries across the African continent. International students have come from Ireland, the U.S. and Thailand.
Many students are sponsored through U.S. government aid programs aimed at reducing disease in Africa or by nongovernmental organizations. Most of the center’s 0,000 annual budget comes from the U.S. President’s Malaria Initiative. Other funding is from the U.S. Defense Department, NGOs and pharmaceutical companies.

For students to practice malaria identification, five Kenyan lab workers work tirelessly to create a variety of blood specimens. Slides may show one or more of malaria’s several species – others are free of parasites. The majority of malaria cases are the falciparum species, but many people are co-infected with other species and it’s important for students to recognize that, Wagar said.

A recent review of the course’s effectiveness showed that microscopy students went back to labs lacking organization and equipment. In some cases, the training was not having the desired impact on local people facing malaria.

“At our course, lab students learn skills and habits that increase their ability to accurately detect malaria on blood slides. Yet, when they return to their local laboratories, they face the challenge of changing habits and procedures,” Wagar said. “Changing behavior is hard to do.”

In late- April, Wagar accompanied Jew Ochola, 28, the center’s daily operations manager to Oyugis, the district center of Rachuonyo that lies roughly 30 miles south of Kisumu in Kenya’s Nyanza province.

The visit marked the initial visit of the center’s supervision support project – monthly visits to six local district hospitals – to implement tools that increase efficient oversight of malaria diagnosis. The yearlong 0,000 initiative – funded by the President’s Malaria Initiative, a USAID sponsored program – is designed to help translate school learning into field practices, Ochola said.

“First I do an assessment of the hospital’s lab, what procedures they have, the number of people on staff and the equipment they use,” Ochola said. “By partnering with laboratory managers, we hope to increase standards and improve efficient and effective diagnosis.
The goal is to lessen the burden of malaria on the local people.”

To mark progress, lab staff must collect 20 slides each month that show properly handled blood samples. Monthly visits will mark performance improvement.
Through quality malaria diagnosis, USAMRU-K is part of a larger public health effort to reduce malaria’s impacts Kenyan’s lives. Illness means paying for treatment and less wages earned, creating an impact on the economy.

“By mitigating a public health burden, people should have more time to grow food and have money for things other than medical care,” Wagar said. “We can’t expect to see change right away, but hopefully things will be a little bit better every month.”

Working with the Djibouti-based Combined Joint Task Force Horn of Africa and other DoD agencies, the center recently offered microscopy courses through U.S. military partnership events in Ghana, Nigeria and Tanzania. The effort supports U.S. Africa Command’s strategic engagement goal of increasing capabilities and strengthening capacity with the militaries of African nations, Wagar said.

“To date, that includes eight Kenyans military lab techs, 17 from the Tanzania People’s Defense Force and 30 Nigerians,” Wagar said.

Accurate diagnosis is also a key factor for military readiness, Wagar said. For example, a Kenyan soldier stationed in Nairobi – where malaria is less prevalent – is susceptible to the disease if posted elsewhere in the country.

“Improving malaria diagnosis within African military laboratories sets conditions for healthier troops,” Wagar said. “When forces are healthy, they are more capable to support their government and regional security.”
